OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  On or about March 4, 2014, employees were exposed crushing/struck-by hazards associated with the use of automotive lifts:  a)  Makeshift wooden blocks placed on support pads were used instead of accessories approved by the manufacturer.  b)  The left rear safety dog was broken on the post of the PMW 12,000 lb 4 post lift.   c) The right rear outside arm of the PMW 7,000 lb. hydraulic lift next to the wall is bent downward where the fork lift caught it.  d)  Lift operators were not trained on the hazards associated with lifts, how to safely operate the lifts, and the method for safely securing vehicles on lifts.  e)  The employer did not develop a periodic inspection procedure in accordance with the manufacturer's recommendations, with documentation of each periodic inspection.  f)  The employer did not develop a periodic planned maintenance procedure in accordance with the maunfacturer's recommendations, with a record of each planned maintenance procedure performed.
