The fire extinguisher was located near the doorway of Unit #22 trailer, found at the Orion Rig#2 drilling site.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about February 28, 2014 the 208 Volt AC power cord connecting Unit 22 to the rig transformer had a strain relief device that was defective.  The plug was located at the 75 KVA transformer providing trailers with power, found on the Orion Rig#2 location.  This condition created an electrical shock hazard at the oil well drilling location.
