29 CFR 1926.351(b)(2): Welding cable(s) in use were not free from improper repair or splices for a minimum distance of 10 feet from the cable end to which the electrode holder was connected: a.) Weld Technology of Virginia, LLC., Hampton VA Medical Center, Police Service Building 36: On or about 03/05/14, an employee welding a handrail on the outside stairs was using a welding cable that had a repair 16" from the electrode holder, creating a contact with electrical current hazard.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a.) Weld Technology Of Virginia, LLC., Hampton VA Medical Center, Police Service Building 36: On or about 03/05/14, an employee was welding on stairs with an unprotected edge approximately 10.5' above the ground without the use of a fall protection system, creating a fall from height hazard.
