29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a. On or about February 28, 2014, in the warehouse, employees retrieving stock items were exposed to 5 to 14 feet fall hazards from the upper tiers of the racking system, in that the edges were not protected by guard rails.

29 CFR 1910.36(g)(4): Objects that projected into the exit route reduced the width of the exit route to less than the minimum width requirements for exit routes: a. On or about February 28, 2014, in the warehouse, the width exit route from the back end of aisle A-B was reduced to 27 inches by the placement of rows of palletize boxes in the aisle.
