The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) House roof at 619 Towsend Place, Powder Springs, GA: An employee was exposed to fall hazards while using an Ascension positioning belt/mountain climbing with rope grab located at the wearer's waist on a combined 10:12 and 14:12 pitched roof at heights of approximately 14 feet from the eave to the lower level.

29 CFR 1926.1053(b)(1):  A portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist emmployees in mounting and dismounting the ladder.  a) At the roof of a house at 619 Towsend Place, Powder Springs, GA- The employer was using an extension ladder to access the roof of a single family house that was not extended three (3) feet above the landing exposing the employees to fall hazards.
