29 CFR 1910.178(l)(4)(iii): An evaluation of each powered industrial truck operator's performance was not being conducted at least once every three years: The employer is failing to protect employees from potential struck by hazards associated with failing to evaluate and recertify forklift operators every three years. This was most recently evident during the inspection of the grain handling facility located at 6040 Fletcher Ave, Lincoln, NE 68517, where the employer has employees operating a forklift as a means of moving material and product throughout the facility without having maintained a current certification for the operators; allowing certifications for employees to expire in January 2014 and the recertification class did not take place until March 3, 2014.
