While the cover is not in place, the pit or trap opening shall be constantly attended by someone or shall be protected on all exposed sides by removable standard railings.   a)      Valley Automotive and Diesel at 120 South Columbine Court, Parachute, CO 81635: On and before 2/28/14, the employer did not ensure that every pit and trapdoor floor opening, was guarded by a floor opening cover of standard strength and construction. This condition exposed employees to a fall hazard of 65 inches.   Abatement Note:  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
