Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    a. On or about March 05, 2014, at the above addressed job site, employees were exposed to the hazard of falling while working on the roof of a commercial building without a fall protection system in place.
