29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  (a) Studer-Obringer on or about February 26, 2014, at the address of 800 West Main St. Woodville, OH: The employer failed to ensure a ladder or similar appropriate means of access was provided to employees accessing and/or egressing from a mast climbing scaffold system. Employees were exposed to a fall of approximately 18 ft. to the ground below when utilizing the crossbracing (web) of a mast climbing scaffold tower to access the platform above.

Employees using the supporting tower of a mast climbing scaffold for access/egreee were required to transition more than 30 in. horizontal from the towers crossbracing (webbing) to the working platform. Employees were exposed to an 18 ft. fall to the ground below.
