29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: no guard was present: On or about 3/4/2014, along the south wall of the manual lathe area, the employer allowed employees to operate an abrasive wheel with a missing tongue guard in order to sharpen bits used in milling operations.

29 CFR 1910.242(b): Compressed air used for cleaning purposes was not reduced to less than 30 p.s.i.: 1) On or about 3/4/14, in the welding shop area near the manual lathe, the employer allowed an employee to operate an air nozzle with a pressure above 30 psig in order to clean debris off of work. 2) On or about 3/4/2014, in the manual lathe area adjacent the welding shop, the employer allowed an employee to operate an air nozzle with a pressure above 30 psig in order to clean debris off of work.
