29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminants chemical state and physical form:   On or about 3/4/2014, at the facility located at 4912 Marina Road in New Iberia, LA, the employer failed to conduct a hazard assessment to determine if the respirators worn by welders in confined spaces and enclosed areas were adequate to prevent overexposure.
