29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached), where the outside bottom edge of the wall opening was 6 feet (1.8 m) or more above lower levels and the inside bottom edge of the wall opening is less than 39 inches (1.0 M) above the walking/working surface, is not protected from falling by the use of a guardrail system, a safety net system, or a personal fall arrest system:  a.   On or about March 5, 2014, on the north side of the house, a guardrail system was not in place around a second level wall opening, exposing employees to a 13 feet 9 inch fall hazard.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a.   On or about March 5, 2014, on the south side of the house, employees working beneath employees working on a framed roof were not wearing hard hats, exposing themselves to the hazard of being struck by falling debris.    Rivas Rivas Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.100(a), which was contained in OSHA inspection number 950950 citationn number 01, item number 01 and was affirmed as a final order on January 24, 2014, with respect to a workplace located at 58 Bridgewater Ln, Lot #19, Watersound, FL 32413.  The Rivas Rivas was previously cited for a violation of this occupational safety and health standard or its equivalent standard  1926.100(a), which was contained in OSHA inspection number 950966, citation number 01, item number 01 and was affirmed as a final order on Janaury 24, 2014, with respect to a workplace located at 39 Wisteria Way, Santa Rosa Beach, FL, 32459.

On or about March 5, 2014, on west side of the house, two employees building a frame structure for a roof were not protected from a 25 feet fall hazard by the use of a personal fall arrest system.  Rivas Rivas Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 950950, citation number 01, item number 02 and was affirmed as a final order on Janauary 24, 2014, with respect to a workplace located at 58 Bridgewater Ln., Lot #19, Watersound, FL 32413. 29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a.   On or about March 5, 2014 on the north side of the house, the ladder used to access the roof did not extend 3 feet above the upper landing surface, exposing employees to a 14 feet fall hazard.
