29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:    a) On or about 03/05/14 - Single jack department, an employee operating single jack machines was exposed to occupational noise at 177% of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of 94.1 dBA. The sampling was performed for 460 minutes on 03/12/14.  Zero percent exposure was assumed for the rest of the shift.    b) On or about 03/05/14 - Single jack department, an employee operating single jack machines was exposed to occupational noise at 134.3% of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of 92.1 dBA. The sampling was performed for 460 minutes on 03/12/14.  Zero percent exposure was assumed for the rest of the shift.    c) On or about 03/05/14 - Plating line department, an employee operating a plating line was exposed to occupational noise at 137.2% of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of 92.2 dBA. The sampling was performed for 446 minutes on 03/12/14.  Zero percent exposure was assumed for the rest of the shift.    d) On or about 03/05/14 - Sash Department, an employee operating small sash machines was exposed to occupational noise at 121.1% of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of 91.3 dBA. The sampling was performed for 468 minutes on 03/12/14.  Zero percent exposure was assumed for the rest of the shift.    e) On or about 03/05/14 - Plating line department, an employee operating a plating line was exposed to occupational noise at 108.1 % of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of 90.5 dBA. The sampling was performed for 450 minutes on 03/12/14.  Zero percent exposure was assumed for the rest of the shift.    f) On or about 03/05/14 - Double loop department, an employee operating double loop machines was exposed to occupational noise at 76.67 % of the allowable 8-hour time-weighted average of 85 dBA or an equivalent sound level of 88.0 dBA. The sampling was performed for 449 minutes on 03/12/14.  Zero percent exposure was assumed for the rest of the shift.
