29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.   (a) North Side of Building - On or about March 5, 2014 employees were exposed to fall hazards while using wood plank scaffolds to install Tyvek Home Wrap. Employees were not protected from falling 15 feet to a lower level by a guardrail system or personal fall arrest system.

29 CFR 1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:Note:  If the employer could demonstrate that it was infeasible or created a greater hazard to use these systems, a fall protection plan meeting the requirements of paragraph (k) of 1926.502 should have been developed and implemented.   (a)  Southeast Corner of Building - On or about March 5, 2014 employees were exposed to fall hazards over 6 feet while installing leading edge plywood. Employees were not protected from falling 14 feet to a lower level by personal fall arrest systems or guardrail systems or safety net systems.

Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws.   (a) East Side of Building - On or about March 5, 2014 employees were exposed to electric shock hazard while using flexible cords without undamaged outer insulation forming strain relief for the cords connectors.
