29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: On or about March 6, 2014; on the south side of the jobsite, the employer allowed employees to utilize extension cords in a manner that was not in accordance with instructions included in the labeling. Employees connected extension cords together to create a chain of two (2) extension cords in order to power a hand-held circular saw.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: On or about March 6, 2014; on the south side of the jobsite, the employer allowed employees to utilize an extension cord that was missing a ground pin. The extension cord was used to energize a hand-held circular saw used to cut and remove a section of wall. The missing ground pin prevented the extension cord from having a continuous path to ground.
