29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  a) The employer failed to provide documentation and certification for abatement of citation 1 item 1 in which employees were not provided guardrail systems and/or no personal fall arrest systems. Condition observed on or about 03/05/14.  b) The employer failed to provide documentation and certification for abatement of citation 1 item 2 in which the supported scaffold poles, legs, post, fames, and/or uprights were not plumb and braced to prevent swaying and displacement. Condition observed on or about 03/05/14.  c) The employer failed to provide documentation and certification for abatement of citation 1 item 3 in scaffold platforms above or below a point of access, portable ladders, attachable ladders, or any direct access from other scaffold, structure, personnel hoist or similar surface was not used. Condition observed on or about 03/05/14.  d) The employer failed to provide documentation and certification for abatement of citation 1 item 4 in which scaffolds and scaffold components were not inspected by a competent person. Condition observed on or about03/05/14.  e) The employer failed to provide documentation and certification for abatement of citations 1 item 6 in which poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary. Condition observed on or about 03/05/14.  f) The employer failed to provide documentation and certification of abatement of citation 1 item 7 in which the employer did not provide scaffold training. Condition observed on or about 03/05/14.  g) The employer failed to provide documentation and certification of abatement of citation 1 item 8 in which ladders were used on slippery surfaces. Condition observed on or about 03/01/14.  h) The employer failed to provide documentation and certification of abatement of citation 2 item 1 in which supported scaffold poles did not bear on the base plates, mud sills or adequate firm foundation. Condition observed on or about 03/05/14.  I) The employer failed to provide documentation and certification of abatement of citation 2 item 2 in which employees were not provided fall protection. Condition observed on or about 03/05/14.
