29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use.  Appropriate eyewash stations were not available in locations including, but not limited to Med Center janitorial closet, Garden Level hopper room, and Garden Level janitorial closet. Employee(s) in these areas were exposed to corrosive cleaners containing chemicals such as hydrogen peroxide, citric acid, and ammonium chlorides.

29 CFR 1910.1030(c)(1)(v): The employer did not document evidence in the Exposure Control Plan of the solicitation of input from non-managerial employees responsible for direct patient care, who were potentially exposed to injuries from contaminated sharps, in the identification, evaluation, and selection of effective engineering and work practice controls.  The facility Exposure Control Plan had not been updated annually and did not contain evidence of employee input and selection of safer medical devices for the control of sharps injuries.
