On or about March 6, 2014, at a residential structure located at 6th & N Maple St, North Little Rock, AR 72114, the employer did not ensure an employee was using personal fall arrest systems while framing a two-story residential structure. An employee was observed/exposed to fall hazards of greater than 20 feet to unfinished ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On or about March 6, 2014 at residential structure located at 6th & N Maple St, North Little Rock, AR 72114, the employer did not ensure employees were properly trained in construction safety while working on residential construction. Employees were observed/exposed to fall hazards greater than 16 feet to finished ground below.
