Unless necessary for a use permitted in paragraph (g)(1)(i) of this section, flexible cords and cables shall not be used: 29 CFR 1926.405(g)(1)(iii)(C): Where run through doorways, windows, or similar openings, except as permitted in paragraph (a)(2)(ii)(1) of this section; a. On the west side of building D where employees were using an extension cord to power an air compressor, the extension cord was ran through a window, thereby exposing employees to a contact hazard.

29 CFR 1926.405(g)(2)(iv): Strain relief. Flexible cords shall be connected to devices and fittings so that strain relief is provided which will prevent pull from being directly transmitted to joints or terminal screws. a. On the west side of building D where employees were using an extension cord to power an air compressor, the strain relief clamp of the male end was pulled away from the outer insulation of the extension cord exposing interior wiring, thereby exposing employees to a contact hazard.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. a. At the east side of building D an employee was lifted up on, worked from, and lowered on a pallet on a forklift, the pallet and forkllift were not designed to hoist personnel, thereby exposing the employee to a 13 foot fall hazard.

On the east side of building D where an employee was working from the elevated forks of a forklift, the employee was not protected from falls of 13 feet, thereby being exposed to fall hazards.
