Workwsite located at 9325 N. Farley Ave., Kansas City, Missouri- Employees were observed working and walking underneath elevated materials and employees who were not wearing any means of head protection.

Worksite located at 9325 N. Farley Ave., Kansas City, Missouri- Employees were exposed to eye injuires when using radial saws and nail guns and were not protected from impact by the use of any means of eye protection.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees:  (a). Worksite located at 9325 N. Farley Ave., Kansas City, Missouri- Employees were exposed to shock or electrocution hazards in that the employer failed to inspect equipment to find cords that had outer insulation breached, cords without ground pins, and cord that had been severed, re-wired and wrapped in electrical tape.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level:   (a). Worksite located at 9325 N. Farley Avenue, Kansas City, Missouri- Employee was observed working on top of a job-made platform in order to frame the roof level over the deck and was not protected from falling by the  use of guardrail systems or personal fall arrest systems.   (b). Worksite located at 9325 N. Farley Avenue, Kansas City, Missouri- Employee was being on a platform that was elevated by a forklift and was not protected from falling by the use of guardrails.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  (a). Worksite located at 9325 N. Farley Ave., Kansas City, Missouri- Employee was exposed to a fall hazard in that the platform front end loader or forklift being used was not designed to lift such a long platform and an employee could end up tipping or breaking the platform.

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel:  (a). Worksite located at 9325 N. Farley Ave., Kansas City, Missouri- Employees were exposed to a fall hazard in that the platform being used by the elevated employee was not secured to the forks to prevent displacement.
