29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.    a) On or about March 6, 2014 at the worksite located at 5574 Cernic Place Austintown, Ohio 44515, two out of the eight employees working on an approximately 45 ft. high 5/12 pitch roof were not protected by any type of conventional fall protection while moving shingles and performing roofing activities.    b) On or about March 6, 2014 at the worksite located at 5574 Cernic Place Austintown, Ohio 44515, eight employees working on an approximately 45 ft. high 5/12 pitch roof, were permitted to unhook their harnesses and rope grabs in order to walk from the north end of the building to the south end of the building, which is approximately 230 ft. long, to wait for an aerial lift to ferry them off the roof.  The employees were not protected from falling while they were walking from the north end to the south end of the roof.     Boak and Sons Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 190116, citation number 2, item number 1 and was affirmed as a final order on 4/2/2012, with respect to a workplace located at 646 Dragon Rd., Niles, OH 44515.    Boak and Sons Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 314456880, citation number 1, item number 2 and was affirmed as a final order on 6/9/2010, with respect to a workplace located at4357 Warren Sharon Rd., Vienna, OH 44473.
