29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  (a). Worksite located at 1904 N. Michigan Ave., Kansas City, Missouri- Employees were exposed to fall hazards while working on the steep roof and were not protected from falling by the use of any means of fall protection.

Worksite located at 1904 N. Michigan Ave., Kansas City, Missouri- Employees were exposed to a fall hazard in that the ladder being used to access the upper landing surface did not extend at least 3 feet above the upper landing surface.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder shall not be used as a step:   (a). Worksite located at 1904 N. Michigan Ave., Kansas City, Missouri- Employees were exposed to fall hazards in that employees were allowed to step on the top two steps of a stepladder in which it could become easily displaced while ascending or descending the ladder.
