29 CFR 1910.252(a)(1)(ii): When the object to be welded or cut could not be moved and all the fire hazards could not be removed, guards were not used to confine the heat, sparks, and slag, and to protect the immovable fire hazards. a) On or about March 10, 2014, at the above located workplace employees were required to arc weld within 35 feet of combustible material which included machine rollers composed of and/or covered with combustible material. The combustible material was not protected with guards to confine the sparks, slag, and to protect immovable fire hazards. The employees were thereby exposed to the hazards associated with fire and smoke inhalation.

Management had not established areas where it was safe to perform cutting and welding without procedures and/or establish, implement and maintain effective procedures for cutting and welding in other areas. Employees were thereby exposed to the hazards associated with fire and smoke inhalation.
