OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish the employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to an employee in that employees were exposed to a fall hazard: a) Facility: On or about 03/07/2014, employees were exposed to fall hazards when picking stock from shelving units in the warehouse that were approximately ten (10) feet in height. Employees stood on wooden pallets while the forks of the Nissan Propane Powered Forklift (M#: MCUG1F2F36LV; S#: F676893); were raised in the air. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
