29 CFR 1926.451(e)(6)(i): Be specifically designed and constructed for use as ladder rungs; On or about March 7, 2014, at 107 E. Market Street, Urbana, a two-story building, employees were gaining access to the scaffold work platform by climbing the scaffold end frame which were not specifcally designed for such use, rung lengths were less than 8", rungs were not uniformly spaced, and exposed employees to a 15 ft. fall hazard.

29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. On or about March 7, 2014, at 107 E. Market Street, Urbana, a two story building, the company did not ensure a competent person inspected the mobile scaffold before the work shift to recognize the lack of square, plumb, possible restraint from tipping and fall protection on the mobile scaffold.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable: On or about March 7, 2014, at 107 E. Market Street, Urbana, a two story building, employees working on a mobile scaffold had not received training on recognizing the hazards and the procedures for controlling or minimizing those hazards.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. On or about March 7, 2014, at 107 E. Market Street, Urbana, a two story brick building, an employee was exposed to a fall hazard while working on a mobile scaffold approximately 15 ft. off the ground without fall protection.
