29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Dairy Specialists LLC at 7001 N. I-76 Frontage Rd, Wiggins, CO 80654: On and before September 24, 2013 machine specific, step-by-step procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On September 24, 2013 an employee's arm was severed when servicing a pit pump that was energized by 3-phase, 460 volts. This condition exposed employees to an amputation hazard. Abatement Note: Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 1) A specific statement of the intended use of the procedures; 2) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; 3) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and 4) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(c)(7)(i)(A): Authorized employees did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: (a) Dairy Specialists LLC at 7001 N. I-76 Frontage Rd, Wiggins, CO 80654: On and before September 24, 2013 the employer did not train authorized employees on machine specific energy control procedures for the control of potentially hazardous energy. On September 24, 2013 an authorized employee's arm was severed when servicing a pit pump that was not de-energized. The pump was energized by 3-phase, 460 volts.
