29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: a) Dietary-Employees are exposed to splash hazards while cleaning the oven and other equipment by spraying chemicals that contain sodium hydroxide, diphosphoric acid, and phosphoric acid classified as corrosive. The emergency eye wash provided was not adequate to flush both eyes simultaneously and the body in case of an emergency, on or about 2/27/14. b) Laundry- Employees are exposed to splash hazards while using washing chemicals that contain sodium hydroxide classified as corrosive. The emergency eye wash provided was not adequate to flush both eyes simultaneously and the body in case of an emergency, on or about 2/27/14. c) Housekeeping-Employees are exposed to splash hazards while cleaning equipment and rooms with spraying chemicals containing sodium hydroxide, potassium hydroxide classified as corrosive. The emergency eye wash provided was not adequate to flush both eyes simultaneously and the body in case of an emergency, on or about 2/27/14.

29 CFR 1910.1200(h)(3)(iv): Employee training did not include the details of the hazard communication program developed by the employer, including an explanation of the labeling system and the material safety data sheet, and how employees can obtain and use the appropriate hazard information: Employees working with hazardous chemicals such as, but not limited to, Solid Stainaway, Solid Surge Plus Achieve, Ally, Bleach, and Lustro, Morning Mist, and bleach were exposed to inhalation and skin contact hazards. The employer failed to ensure all employees were trained on the new label elements (pictograms) and SDS format, on or about 2/27/14. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
