29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by point of operation, ingoing nip point, rotating parts and/or flying chips or sparks: On or about March 10, 2014, the employer did not ensure that employees operating the Cincinnati press brakes were protected from hazards created by the point of operation. a) Employees, including but not limited to, operators, trainers and trainees, were not protected by the hazard created by multiple die setups that include an off-set die and/or hemming die with a gap opening greater than 1/4" between the upper and lower dies. b) Employees were not protected from the hazard of the point of operation when feasible machine guarding, including but not limited to, a presence sensing device (i.e., light curtain) was not active and/or utilized.
