29 CFR 1926.452(w)(3): Manual force used to move the scaffold shall be applied as close to the base as practicable, but not more than 5 feet (1.5 m) above the supporting surface. WORKSITE: The employee, while standing on the working platform of a Perry mobile scaffold which was six (6) feet above the floor was manually moving the scaffold. On or about 3/10/2014, the Perry mobile scaffold from which an employee was working tipped over while the employee was trying to manually move the scaffold from the platform, and the employee fell to the floor as well sustaining serious injuries from the fall.

29 CFR 1926.452(w)(6)(i): The surface on which the scaffold is being moved is within 3 degrees of level, and free of pits, holes, and obstructions; WORKSITE: The concrete floor on which the Perry mobile scaffold was being manually moved by an employee from the working platform was not maintained free of obstructions and/or other debris. On or about 3/10/2014, the wheels of the Perry mobile scaffold caught on a sheet of plywood and the scaffold tipped over while an employee was trying to manually move the scaffold from the platform. The employee fell to the floor as well and sustained seriously injuring from the fall.
