29 CFR 1910.147(c)(2)(ii): The employer's energy control program under paragraph (c)(1) of this section did not utilize lockout, where the energy isolating device was capable of being locked out:    (a)  Asher Brewing Company, at 4699 Nautilus Court #104, Boulder, CO:  On and preceding 3/10/14, Asher Brewing Company did not ensure that the energy control program under paragraph (c)(1) of this section utilized lockout, where the energy isolating device was capable of being locked out.  Employees utilized energy control procedures on equipment including but not limited to when employees cleaned process equipment.  Process equipment, including the mash tun, brew kettle, and fermentation tank, had multiple energy sources, including electrical, pressurized water, pressurized chemical, and thermal.  The employer's energy control plan utilized tags when closing valves or turning off circuit breakers to isolate an energy source.  The valves and breakers were capable of utilizing lockout.  The employer did not ensure that lockout was used when closing valves or turning off circuit breakers to isolate an energy source.  This condition exposed employees to uncontrolled hazardous energy sources.

29 CFR 1910.147(c)(4)(ii)(B):  The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy:    (a)  Asher Brewing Company, at 4699 Nautilus Court #104, Boulder, CO:  On and preceding 3/10/14, Asher Brewing Company did not ensure that the energy control procedures clearly and specifically outlined the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.  Employees utilized energy control procedures on equipment including but not limited to when employees cleaned process equipment.  Process equipment, including the mash tun, brew kettle, and fermentation tank, had multiple energy sources, including electrical, pressurized water, pressurized chemical, and thermal.  The employer's energy control procedure for the mash tun and brew kettle did not contain specific procedures for the de-energization and control of the hazardous energy sources.  This condition exposed employees to uncontrolled hazardous energy sources.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:    (a)  Asher Brewing Company, at 4699 Nautilus Court #104, Boulder, CO:  On and preceding 3/10/14, Asher Brewing Company did not ensure that where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were provided within the work area for immediate emergency use.  Employees used BIRKO Corporation Acid Brite #2 (contains 6% nitric acid, 26% phosphoric acid) and BIRKO Corporation Ultra Niter (contains 12% nitric acid, 4% phosphoric acid) when cleaning process equipment.  Nitric acid and phosphoric acid are corrosive liquids.  An emergency eyewash or shower were not located in the work area.  This condition exposed employees to a corrosive liquid hazard.

Employees utilized energy control procedures on equipment including but not limited to when employees cleaned process equipment.  Process equipment, including the mash tun, brew kettle, and fermentation tank, had multiple energy sources, including electrical, pressurized water, pressurized chemical, and thermal.  The employer did not certify that employee training had been accomplished.  This condition potentially exposed employees to uncontrolled hazardous energy sources.
