29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    Location: 28 Merrel Avenue, Stamford, CT 06902    The employees who were working on the third floor at a residential construction site were not protected from falling into open holes on the third floor.

Location: 28 Merrel Avenue, Stamford, CT 06902    Employees working on the second floor at a residential construction site were not protected from falling out a wall opening where the opening did not have a top rail guardrail.

29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge:    Location: 28 Merrel Avenue, Stamford, CT 06902    An employee who was working on the third floor at a residential construction site was not protected from falling where the guardrail system failed when it was leaned against resulting in a 21 foot fall to ground.
