29 CFR 1910.37(b)(4): Signs were not posted along the exit access indicating the direction of travel to the nearest exit and exit discharge when the direction of travel to the exit or exit discharge was not immediately apparent: a) At the warehouse of KSI Trading Corporation located at 4763 Oak Fair Blvd., Tampa, FL 33610 as observed on or about 03-11-2014. The emergency exit sign was blocked by boxes stored on the shelf that surrounds the emergency exit door exposing employees to a fire hazard. b) At the warehouse of KSI Trading Corporation located at 4763 Oak Fair Blvd., Tampa, FL 33610 as observed on or about 03-11-2014. There were no directional arrows posted directing employees to the emergency exit which was surrounded by shelving blocking the view of the exit.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) At the warehouse of KSI Trading Corporation located at 4763 Oak Fair Blvd., Tampa, FL 33610 as observed on or about 03-11-2014. In that the D-ring for the fall protection harness used by the operator of the Prime Mover (OPX30, Electric Counterbalanced Order Picker) was connected to the anchor point improperly. The gate of the D-ring was positioned such as to make it the load bearing member exposing them to a fall hazard. b) At the warehouse of KSI Trading Corporation located at 4763 Oak Fair Blvd., Tampa, FL 33610 as observed on or about 03-11-2014. The Crown SP3000 was being used without fall protection while being operated exposing employees to a fall hazard and crushed by hazard.
