29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: (a) Big Tuna Yard: Garibaldi Drydock Inc. installed a fifth wheel attachment to the forks of the Allis Chalmers forklift serial number 59259. This exposed the employee to potential struck-by hazards from lifting loads that the forklift was not designed to lift.
