29 CFR 1910.212(a)(3)(ii): Points of operation of machinery were not guarded to prevent employees from having any part of their body in the danger zone during operating cycles: a.) On 3/6/2014, a line operator was hand feeding plastic stock into a Bosch GCM12SD Jewel Bevel Hinge Slide mitre saw, SN 202001474, and was struck in his thumb by the rotating saw blade. The saw was equipped with a retracting guard over the lower section of the blade, which left 2 inches to 3 inches of rotating blade exposed during usage. The employee suffered a de-gloving and fracture of his left hand thumb. Abatement Certification and Documentation is required.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: a.) On and before 3/12/2014, the company had not developed a written program that described: the person designated as responsible for assuring and maintaining labeling, Safety Data Sheets (SDSs) and training requirements. Written materials did not include procedures for these elements, a description of non-routine tasks nor the means to inform or be informed of hazards related to outside contractors.
