29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a) 2201 South 22nd Street, Philadelphia, PA - 2201 South 22nd Street, Philadelphia, PA - The employee was installing counter flashing from an elevated platform which did not have any end rails installed. The employee was approximately 93.5 feet above the ground. This hazard exposed the employee to a fall hazard greater than 10 feet. This condition was observed on or about February 21, 2014.
