29 CFR 1926.451(b)(1): 29 CFR 1926.450(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: The employer does not ensure each platform on all working levels of scaffolds were fully planked or decked. This violation was observed on or about February 19, 2014, at the job site, where an employee was exposed to a fall hazard of more than 10 feet while installing insulation on the school building without the scaffold platform being fully planked. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that each platform on all working levels of scaffolds was fully planked or decked.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. The employer does not ensure that employees working from a scaffold more than then 10 feet above a lower level is protected from falling. This violation was observed on or about February 19, 2014, at the job site, where employees were exposed to a fall hazard of more than 10 feet while installing insulation on the school building without the use of a personal fall arrest system or a guardrail system. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when working on a scaffold 10 feet above a lower level.
