29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a) At the South end of the trench where underground utilities were being installed, employees working under the bucket of the excavator were not wearing hard hats to protect them from being struck-by the debris or materials as observed on or about March 5, 2014. b) At the North end of the trench where underground utilities were being installed, an employee working under the bucket of the excavator was not wearing a hard hat to protect him from being struck-by the debris or materials as observed on or about March 5, 2014.
