29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification: A) Worksite - On or about March 11, 2014, a standard 120V electrical box with 1 duplex receptacle, which was designed to be mounted on a wall, was hanging from a flexible cord and was used to provide electricity to the cement mixer. B) Worksite - On or about March 11, 2014, a standard 120 V electrical box with 2 duplex receptacles was hanging from a piece of armored cable. This box supplied electricity to the previously mentioned box that powered the cement mixer.
