29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (A)-(D) of this section:  a)  Throughout Facility - On or about March 12, 2014, specific step-by-step procedures were not developed for the control of hazardous energy on equipment, to include but not limited to Pouch Machines and Printing Presses, on which employees perform servicing and maintenance, including equipment or machine set up.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:   (a)  Throughout Facility - On or about March 12, 2014, employees who perform servicing and maintenance on equipment and machinery with hazardous energy sources were not provided with training to the authorized level, in that they were not trained in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   (b)  Throughout Facility - On or about March 12, 2014, operators of equipment and machinery with hazardous energy sources were not provided with training to the affected level, in that they were not instructed in the purpose and use of the energy control procedure.   (c)  Throughout Facility - On or about March 12, 2014, all other employees whose work operations are or may be in an area where energy control procedures may be utilized, were not instructed about the procedure, and about the prohibition relating to attempts to restart or re-energize machines or equipment which are locked out or tagged out.

29 CFR 1910.212(a)(2):     Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:    (a)  Production Area - On or about March 12, 2014, employees were exposed to hazards created by ingoing nip points on the Pouch Machines, in that the bottom sections were not guarded allowing employee access to the Rear Nip Roller while the machine is in operation.    (b)  Production Area - On or about March 12, 2014, employees were exposed to hazards created by moving parts on the Pouch Machines, in that presence sensing devices (light curtains) were bypassed to allow operators to reach into the machine while in operation

(b)  Printing Area - On or about March 12, 2014, printing press operators were exposed to nip point hazards created by print cylinders on the Mark Andy Flexo Graphic Printing Press.
