OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and fire hazards from the improper rigging of fuel gas cylinders: Jobsite: Instance A: While hoisting fuel gas cylinders, the employees are securing sling hooks directly to cylinder collars. One feasible abatement method, among others, is to use a cradle or platform for lifting the cylinders. Instance B: On or about May 2, 2014, a 1000 gallon tank was lifted by a chain sling using a sling angle not authorized by the sling manufacturer. One feasible abatement method is to follow the manufacturers' instructions regarding use of chain slings.
