29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about 3/10/2014, Clybourn Room, Zone 2: Clybourn packaging machine VCTN2 was not equipped with machine guarding to prevent contact with a rotating table with pinch points and rotating metal "tuckers", which created shear points. b) On or about 3/10/2014, Clybourn Room, Zone 2: Clybourn packaging machine VCTN1 was not equipped with machine guarding to prevent contact with unguarded cams and rollers, oscillating metal parts, moving product holders and rotating metal "tuckers" which created shear points. c) On or about 3/10/2014, In the Canning Room: Canning Machine #1 was equipped with a rigid plastic conveyor which had an unguarded pinch point at its end. d) On or about 3/10/2014, In the Mixing Area: A large Ribbon Mixer/ Blender had inoperable interlocks for its removable metal mesh inserts. e) On or about 3/10/2014, In the Gluten Free Room, Zone 4: The out feed conveyor for the sewing machine had a portion that was not guarded at its roller. f) On or about 3/10/2014, In the Gluten Free Room, Zone 4: The out feed conveyor for Bag and Box machine #2 had damaged metal lacing and an unguarded pinch point at its drive drum. g) On or about 3/10/2014, In the Gluten Free Room, Zone 4: Clybourn machine #1 was not equipped with machine guarding to prevent contact with unguarded rotating metal "tuckers" that created shear points. h) On or about 3/10/2014, In the Gluten Free Room, Zone 4: The Bag and Seal machine had an unguarded vertical sealer bar. i) On or about 3/10/2014, In the Gluten Free Room, Zone 4: THe Bag and Seal machine had an inadequately guarded clamper and cutter operation. j) On or about 3/10/2014, In the Gluten Free Room, Zone 4: The out feed conveyor from the Bag and Seal machine had unguarded rollers on the side of the conveyor frame. k) On or about 3/10/2014, In a Gluten Free Area: Holes were cut into the door of the Bagger Machine #1. l) On or about 3/10/2014, In a Gluten Free Area: The in feed conveyor for the Elliot Cartoner had unguarded shear points between the metal product holders and the back portion of the conveyor frame. m) On or about 3/10/2014, In a Gluten Free Area: The in feed conveyor for the Elliot Cartoner had conveyor slats missing, which created pinch points. n) On or about 3/10/2014, In a Gluten Free Area: The Elliot Cartoner had an end guard missing,exposing moving parts. o) On or about 3/10/2014, Trolley Area: No guarding was provided to prevent contact with the wheels of the trolley and the trolley.
