On or about March 11, 2014, on the east side of the construction site, employees were conducting steel erection activities above protruding rebar to which they could fall on, exposing employees to impalement, and death.

29 CFR 1926.753(d)(2)(ii): Hooks with self closing safety latches were not used to prevent components from slipping out of the hook:  a.  On or about March 11, 2014, on the east side of the structure, employees were rigging loads while using a hook that had a broken safety latch, employees below were not protected from struck-by hazards.

29 CFR 1926.251(a)(2)(iii): The employer did not ensure the sling tag indicating the safe working load was present:  a.   On or about March 11, 2014, on the east side of the  construction site, employees were using slings that did not have legible identification markings as prescribed by the manufacturer that indicate the recommended safe working load, exposing employees to struck-by hazards.    b.  On or about March 11, 2014, on the east side of the construction site, employees were using cable straps that did not have an affixed identification marking as prescribed by the manufacturer that indicate the recommended safe working load, exposing employees to struck-by hazards.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems:  a.   On or about March 11, 2014, on southeast side of the structure, an employee welding studs and vent plates to the frame structure was not protected from a 16 feet fall hazard by the use of a personal fall arrest system.

29 CFR 1926.753(c)(2):  A qualified rigger shall inspect the rigging prior to each shift in accordance with 29 CFR 1926.251:  a.   On or about March 11, 2014, on the above addressed jobsite, the employer did not ensure the rigger was qualified who could identify slings and fasteners for defects and remove them from service if they were found to have deficiencies.
