29 CFR 1926.451(b)(3): The front edge of the platform was more than 14 inches from face of the work with no guardrail systems erected along front edge nor was a personal fall arrest system used to protect employees from falling: a) 1033 Arden Avenue, Staten Island, NY 10312. The employee was working on and from Alum-A-Pole pump jack scaffold when the Alum-A-Scaff (aka platform) was more than 14 inches away from the face of the work and no guardrail systems erected along front edge nor was a personal fall arrest used to protect the employee from falling. Condition occurred on or about 03/12/14.
