29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: In the shipping and receiving area on or about March 12, 2014, and at times prior thereto, there was no eyewash in the battery charging area for the Yale pallet jack. It would take more than 10 seconds to get to the nearest eyewash as it was located about 150 feet away in the facilities shop. To get to the eyewash, employees needed to go the half the length of the shipping and receiving area, out a side-hinged exterior door, down four stairs, across an asphalt driveway, and through a curtained roll-up door entrance to the facilities shop.

Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.: In the facilities shop on or about March 12, 2014, and at times prior thereto, there was no guard for the chuck on a Rigid Pedestal Drill Press, model number DP15000, serial number 99177E0098. The drill press was used to drill holes in sheet metal for the HVAC system.

29 CFR 1910.213(p)(4): Belt sanding machines were not provided with guards at each nip point where the sanding belt runs on to a pulley. These guards did not effectively prevent the hands or fingers of the operator from coming in contact with the nip points: In the facilities shop on or about March 12, 2014, and at times prior thereto, the ingoing nip points between the sand paper belt and rollers were not guarded on a Dayton 6x12 inch Belt and Disc Sander, model number 4TJ97B, lot no. 3007. There was a three-eights inch gap between the 6 inch wide belt and the 3 inch diameter pulley. The nip point was 2 feet, 10 inches above the floor.
