29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:      On or about March 11, 2014, at residential construction located at 8000 River Pointe Drive, Maumelle, Arkansas 72113, the employer did not ensure an employee properly wore a personal fall arrest systems while installing CMU concrete blocks, stucco and rock on exterior walls of residential building structure. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level or above the wearer's head. The employee was exposed to fall hazards from an elevation greater than 25 feet to the unfinished ground level.
