29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:  a) On or about March 11, 2014, and at times prior, employees had not been trained in the recognition and avoidance of the hazards associated with trenching and excavations.  b) On or about March 11, 2014, and at times prior, employees were not trained in the use and understanding of gas monitoring equipment.

29 CFR 1926.651(b)(1): The estimated location of underground utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, were not determined prior to opening the excavation:  a)  On or about March 11, 2014, and at times prior, employees were exposed to fire and explosion hazards where the location of an underground gas line was not determined prior to opening a trench for the installation of a water line.  A rupture in the gas line lead to an explosion which injured employees.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions:   a)  On or about March 11, 2014, and at times prior, employees were exposed to fire and explosion hazards where following the rupturing and subsequent repair of a gas line, no inspection of the gas line for additional ruptures or leaks was peformed, and no air monitoring was performed to determine the levels of gas accumulation in the work area.
