29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) An employee was instructed, by his supervisor, how to correct a slipping conveyor belt by removing the guard and pulling downward on the conveyor drive belt. An amputation injury occurred when the employee removed the guard and pulled against the drive belt. The conveyor started and caught the employee's left thumb between the drive belt and drive pulley, amputating the thumb below the nail. Abatement verification is not required for this item.
