29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     (a) Facility - 198 Meadowlands Boulevard, Washington, PA: On or about March 11, 2014, the employer did not provide a written lock out/tag out program consisting of energy control procedures, or employee training. Employees were locking/tagging out equipment such as but not limited to the Summit Horizontal Boaring Mill to provide maintenance activities.

(b) Facility - 198 Meadowlands Boulevard, Washington, PA: On or about March 11, 2014, an employee operating the DoAll Bandsaw was exposed to 10.5" of exposed blade due to the lack of guarding.         (c) Facility - 198 Meadowlands Boulevard, Washington, PA: On or about March 11, 2014, an employee amputated his arm operating the Summit horizontal Boaring Mill when he was exposed to the rotating parts.
