29 CFR 1926.451(g)(1)(vii): For all scaffolds not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, each employee was not protected by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section. 39 New London Turnpike, Glastonbury, CT 06033 on or about 03/11/2014: An employee was walking /working on the top level of a fabricated metal scaffold without being provided with any type of fall protection. The employee was exposed to a 13 foot fall.

The ladder was being utilized to access a work platform on a prefabricated steel scaffold 13 feet in height.
