29 CFR 1910.305(g)(1)(iv)(A): Flexible cords were used as a substitute for the fixed wiring of a structure: a. First Floor - A flexible cord was wired into a metal junction box that was mounted on the ceiling. This cord was plugged into a flexible extension cord that was plugged into a second flexible extension which was plugged into a wall-mounted electrical receptacle outlet. The junction box supplied electric power to a heater. This condition exposed the employees to electrical shock hazards. Observed on or about April 4, 2014

First and Second Floors - The employer did not provide employees with effective information and training on hazardous chemicals in their work area. Employees regularly worked materials such as, but not limited to, fibrous glass, diethylene glycol, tin and silver solder, solder flux, white mineral oil, Mobil DTE-24 Hydraulic Oil, and compressed argon. Employees were potentially exposed to respiratory irritation, contact burns and irritation to the skin. Observed on or about April 4, 2014
