This cord was plugged into a flexible extension cord that was plugged into a second flexible extension which was plugged into a wall-mounted electrical receptacle outlet.  The junction box supplied electric power to a heater.  This condition exposed the employees to electrical shock hazards.  Observed on or about April 4, 2014
